A 24-year-old man presented with abdominal pain, fever and jaundice. Ultrasound examination of the abdomen showed a lithiasis of the gallbladder and a dilatation of the common bile duct without stones inside. A MR cholangiography showed a stenosis in the distal tract of the common bile duct which was dilated. The same picture was demonstrated by an endoscopic retrograde cholangiopancreatography (ERCP). The patient underwent an explorative laparotomy. No stones were found in the gallbladder, but in its infundibular region a hard nodule was present and located in the thickness of the wall. This manuscript describes the epidemiology of myeloid malignancies (MMs), taking into account the morphological characterisation of these tumours. The t(8;21) chromosomal translocation activates aberrant expression of the AML1-ETO (AE) fusion protein and is commonly associated with core binding factor acute myeloid leukaemia (CBF AML). Combining a conditional mouse model that closely resembles the slow evolution and the mosaic AE expression pattern of human t(8;21) CBF AML with global transcriptome sequencing, we find that disease progression was characterized by two principal pathogenic mechanisms.
